Y2K Revisions by HCFA

For the 1500 Claim Form-Clarification

Effective 1/4/99

In 1998 HCFA published guidelines that became effective 10/1/98 for changes to the HCFA 1500.

If a provider completes items 3, 9b, and 11a, Y2K specifications ask that an 8-digit date be provided.

The ORIGINAL Y2K requirements for the 1500 that were published in 1998 included an option for reporting 6 or 8-digit dates in non-birth date fields. If a provider chooses to enter 8-digit dates for items 11b, 12, 14, 16, 18,19, 24a or 31, he or she is instructed to enter 8-digit dates for all these items. Providers should not mix 6 and 8 digit dates in these fields. Effective Jan. 4, 1999 HCFA has made the following change: "The Health Care Financing Administration (HCFA) has decided, after careful consideration, to exempt items 12 and 31 from the above requirements."
