Topic: Optimizing Reimbursement Through Compliant Practices for Revenue Integrity
Medicare outpatient reimbursement depends on a hospital’s methodology for capturing the appropriate charge represented by the correct code(s) charging for the “reimbursable” service in the proper setting! And, now that Medicaid reimbursement requirements often conflict with Medicare – how does your facility ensure appropriate charging, coding and billing guidance is followed by payer? Commercial payers are adopting “their” version of Medicare requirements – how are these defined per contract and how are they met for optimum reimbursement? Although facilities have focused on “charge capture” for several years, 2010 and 2011 present new challenges. Join our detailed review of best practices and updated procedures to ensure optimum – but compliant – reimbursement as we move into the new year.
What you will learn
After completion of the session, the participant will be able to:
- Discuss the CMS provider agreement requirements for determination of medical necessity before providing the service.
- Understand the flow of clinical data necessary to support and earn optimum reimbursement.
- Know the five “do’s” and “don’t’s” regarding 2010 and 2011 charge capture and revenue integrity.
- Detail the best practice checkpoints for billing and account resolution to increase payment.
- Understand why reimbursement is a hospital-wide responsibility – not just PFS or the Revenue Cycle team.

Topic: Implementation of new statutory provision pertaining to Medicare 3-day payment window
On June 25, 2010, President Obama signed into law the “Preservation of Access to Care for Medicare Beneficiaries and Pension Relief Act of 2010.” A provision in the law pertains to Medicare’s policy for payment of outpatient services provided on either the date of a beneficiary’s admission or during the three calendar days immediately preceding the date of a beneficiary’s inpatient admission. The new language changes a long-standing procedure for combining only diagnostic outpatient services and unrelated non-diagnostic (or therapeutic) services based on matching principal diagnoses. Find out how your facility should revise current billing practices to comply with the provision requirements and what financial impact will result.
What you will learn
After completion of this session, a participant will be able to:
- Understand the changes in the language for Medicare’s 3-day payment window
- Discuss the process for determining whether or not outpatient services should be combined with an inpatient claim
- Plan for appropriate review of outpatient services and put in place a billing procedure to meet the new law’s provision
- Determine the financial (payment) reduction as result of combining 3-day outpatient services to inpatient claims

Topic: Implementation of HIPAA 5010 brings changes in claims data requirements
The current HIPAA Transaction Code Set 4010 standard is widely recognized as outdated and lacking in the functionality currently needed by the health care industry. Version 5010 will correct the outdated transaction standard and enhance administrative data exchanges. To accomplish the improvements, the electronic claim formats, entitled 837 institutional and professional transactions, will undergo significant changes. The new level of detail under HIPAA 5010 requires specific revisions in claims data for appropriate processing and payment. This session will discuss changing billing requirements and best practice procedures for implementation of 5010.

- Plan for required billing data element revisions
- Determine source of additional billing information
- Discuss the important components required for implementation of 5010

Linda J. Corley, MBA, CPC, is Dell Services Corporate Compliance Officer for Revenue Cycle Solutions (RCS). She has more than twenty-five years experience working directly for or with hospitals in the areas of Patient Financial Services, Coding and Accounting. An experienced hospital accounting manager and accomplished college professor, Linda has twenty years experience working with financial accounting systems in hospitals and more than twenty-five years experience in training accounting professionals, patient financial services staff and coders for the medical environment. Since joining Dell Services, she has directed more than one hundred Charge Description Master Review projects and has performed numerous coding and compliance audits. She currently leads the Learning and Development team for RCS in addition to her Compliance Officer role. Linda routinely presents Compliance and Revenue Cycle updates to associates as well as coverage, coding, billing and collections training. She has been an integral part of the Revenue Cycle Solutions group for more than twelve years. Linda is a frequent HFMA speaker and is known throughout the U.S. for her lively and interactive, yet fact filled, hospital financial management presentations.

Greetings from the Georgia Chapter of HFMA! Many of you are already involved with Georgia HFMA and aware of the wonderful resource HFMA provides to its members for networking and for staying on top of issues and challenges the healthcare industry presents. HFMA is a national organization of approximately 35,000 healthcare professionals in 68 Chapters around the country. Georgia boasts one of the largest Chapters at approximately 1,260 members. One of my goals this year as the Chapter President is to "invite more people to the table" to obtain all of the benefits I feel strongly HFMA has to offer. One of those areas is Compliance. It is a critical, fundamental, and necessary part of any successful healthcare organization. Most of you deal with Compliance issues every day. Other departments don't deal with Compliance issues quite as frequently. Regardless, they need to be sure they are making decisions and applying processes that are within the guidelines of the organization's compliance policy. Lack of attention to such details can be costly.
So why C.A.R.E.? C.A.R.E. is an acronym for Compliance, Audit, Risk and Ethics. The idea was suggested to me by a former member of the New Jersey Chapter of HFMA. The purpose of the C.A.R.E. Committee is to provide educational opportunities for Georgia compliance, audit, risk and ethics professionals and leaders in the healthcare industry. Leaders and professionals in these areas include, but are not limited to, Directors of Compliance/Compliance Officers, Compliance Auditors/Analysts, Audit Directors/Managers, Internal Auditors, Risk Managers, and Ethics Officers. It is imperative that we encourage collaboration between Compliance and the Revenue Cycle, Information Systems, and Patient Access departments among others.
Marge Scott, Linda Corley, and Kelly Dziedzic have offered to help me bring this Committee to life in the Georgia Chapter. I thank them for their leadership and ideas. We realize there are other resources available to you around the state through HCCA and GHA. It is not our intention to duplicate or replace what these groups are doing. Rather we want to expand the dialogue and collaboration throughout the state and provide additional venues for healthcare financial and compliance professions to network and interact as we work collectively with HCCA and GHA.
We would like to invite you to our first such educational forum. We hope you will make plans to join us for this outstanding event. We will be hosting another Education Forum in early 2011.

Georgia Scroll Article Submission
Become a published author and accumulate Founders Points!
Did you know that you receive two Founders Points for every published article in a Local Chapter or National HFMA publication? There are two types of articles published in the Georgia Scroll. The first are "columns" submitted by various committees as established in the deliverables of the committees. Some committees require submission for every issue; others require two articles per year. When submitting an article on behalf of a committee, please identify the committee and the author(s) in case questions arise.
The second type of articles are those written by various members of the Georgia Chapter, articles from national HFMA, or articles published in other Chapter newsletters or publications. These should be of an educational nature and for purposes of improving the skill level or knowledge base of the readers.
Submission of article: Article should be proof read and edited prior to submission.Submit a sub-caption (heading) for use in the index of the Scroll. Please make sure you name the authors and give a 2-3 line bio at the end of the article.
Send your article by email to jiodice@gwinnettmedicalcenter.org. Name your file in a manner that will be easy to identify the title and the issue for which you are submitting it. Keep in mind that the Editor may receive 15 to 20 or more submissions, if the name of every file is "Scroll Article" or something similar, it will be difficult to differentiate them. Examples of simple file names for columns and articles are:

Title_ Scroll_Winter 09_Author Name
Topic: Write an article in the third person and should not be an advertisement for your company or product. It should contain relevant and informative information for the membership.
When writing the article: Use bullet points and graphs, also insert gray areas (sidebars) with statistical information. Use sub headings when possible.
Ideal Length: 200-400 words. Articles that are longer are accepted and welcomed.

E-Learning Lessons Provided Through HFMA
National HFMA has a wide array of e-learning lessons available through its website. The Georgia Chapter can receive credit for educational hours for each lesson as well as a 20% rebate from National for e-learning lessons purchased through HFMA's website by individuals.
When ordering lessons, just be sure to indicate Georgia Chapter in the space provided (drop down box) on the purchase form . The lessons are affordable for individual purchase or for an organization that needs to train only a few staff members. The price ranges from $99 to $199 per curriculum. The curricula topics are healthcare finance related; billing, accounting and finance, admissions, patient financial services, HIPAA, managed care, physician practice management, medical terminology, compliance and more. Each curriculum has multiple lessons that cover a variety of content for that main topic. The learner can submit for CPE credits by sending the certificate of completion provided to their State Board of Accountancy under self-study.

Or you can go to www.hfma.org and click on education programs.
